About Avanta Home Care
Many patients can benefit from a continuation of the therapy and nursing services they receive at Advanced Health Care facilities. For that reason, we often recommend to patients that are discharging from our facilities to have home health care follow them at home. Avanta Home Care, an Advanced Health Care Company, is our home health care agency.
Avanta Home Care is the leader in providing at-home nursing and rehabilitation services necessary for healthy and independent living. Many of the same staff that works at our facilities also work for Avanta Home Care. Because of this relationship between the facility and Avanta Home Care, patients receive continuity of care with the nursing and therapy treatments as they transition home. This continuation of treatments allows for the fastest recovery time and best outcomes possible. Home care allows therapists, nurses, and nurse assistants, under the supervision of a physician, to perform treatments in patients’ homes at their convenience.
The nursing team at Avanta Home Care works closely with the patient, family, and physician to ensure the individual needs of each person are met. Their licensed nurses develop an individualized plan of care that may include the following services: pain control, wound care, medication management, assistance with dressing and bathing, drawing blood work for lab analysis, assistance with medical equipment, and diabetes education and management.
Physical, Occupational, and Speech therapies are often critical components of a patient’s recovery and plan of care. The goal of Avanta Home Care’s specialized rehab program is to help patients attain their maximum potential and restore the functional abilities and confidence necessary for independent living. Avanta Home Care provides an experienced team of therapists who work with patients in their familiar surroundings at home. Services that may be provided by their therapists include: joint replacement rehabilitation, walking and balance training, strength training, fall and injury prevention, pain management, evaluations for adaptive equipment, training for independence with personal cares, judgment and memory skills, language skills, and swallowing techniques.
Medicare and most other insurances cover home care expenses entirely for as long as there is a need for skilled care that could only be done in the home. Our staff would be happy to answer any questions regarding home care and discuss all options available to our patients.
Rehabilitation
