Derrick Oxnam Administrator
I grew up in Payette, ID and graduated from Idaho State University majoring in finance. I was married in Boise to my wonderful wife Kimberly and am a proud father. I love spending time with my family and can't ever seem to spend enough time outdoors.

Larry Johnson (RN Charge - Administrator-in-Training)
I have a BS in Pre-Med Zoology and BS in Nursing from Idaho State University. I have worked in the healthcare field for 28 years with eight and a half at Grangeville Health & Rehabilitation Center. I am married with ten children and five grandchildren. I love working with the youth in the Boy Scouting program. We have five Eagle Scouts in our family. I love the outdoors.

Phil Mason (Dietary Manager)
I have been the Dietary Manager for Grangeville Health & Rehabilitation Center (and former owners) since 1994. I was born in March 1952. I have two children, a son, Jaime (14 yrs) and a daughter, Jessica (11 yrs). I enjoy horseback riding, four-wheeler riding, boating, and camping.

Ellen Aiken Director of Nursing
Hello from Grangeville, Idaho. My name is Ellen Aiken. I am an RN and the DNS here at GH&R. I was born and raised in Montana, moved to Grangeville in 1976. My husband Ray and I have been married 30 years and have five children (Amy, Ray, Ryan, Steven, and Chelsea). We are the proud grandparents of seven, twins Micaela and Macenzie, Dani, Reed, twins Jordis and Elise, and Kayden. I have worked for Advanced Health Care since 2001. I have been an RN for eleven years, and a CNA many years before that.

Pam Elam (MDS Coordinator)
I have been a LPN for 36 years, mostly in long-term care. I am a married mother of nine children and a grandmother of three girls. Some of my hobbies are travel, cooking, crocheting, visiting my children and grandchildren, and picking wild mushrooms and huckleberries.

Leanne LeBlanc MD (Medical Director)
I am a Louisiana native who moved to Idaho in 1997 to establish a solo family practice. I received my medical degree from Louisiana State University in New Orleans in 1993 and my undergraduate degree from the University of Louisiana in 1986 in "Rajun Cajun" country. I am board certified in family medicine.

Kelly Johnson (Medical Records Director)
I have been the Medical Records Director for Grangeville Health & Rehabilitation Center since 2003. I was born in January, 1967. I have two children, a son, Quinton, and a daughter, Sydney. Some of my hobbies are reading medical books, camping, traveling, writing comedy, and playing with my rat-terrier, Punt.

Lori Castilleja (Housekeeping / Laundry Supervisor)
Hello, my name is Lori Castilleja. I have been working in the Housekeeping / Laundry Department in our facility for twelve years. I have been a Supervisor for five years. I am married, with three children. The experience working for Grangeville Health & Rehab has and will continue to be a wonderful one.

Sandy Cuthbert (Unit Manager)
Hi, my name is Sandy Cuthbert. I have worked in long-term care since I was 16 years old. I graduated from Clarkston School of Nursing as an LPN in 1971. I moved to Grangeville in 1972 and have been in this facility off and on since then. I have worked for Advanced Health Care since 2001. My position is Unit Manager. I am married and have four grown children and five grandchildren.

Tracy Cox (Activities Director)
Let me introduce myself, my name is Tracy Cox, Activities Director for Grangeville Health & Rehabilitation Center. I have worked in health care for over 25 years as a NA/CNA. I began working for Advanced Health Care in 2003. I have been in Activities for one year, a job I truly love. My goal in life is to provide fun and interesting activities, that the residents enjoy, and to make their stay with us a pleasurable experience. I was born and raised in Grangeville, Idaho, and have spent my entire life here. My husband Ken and I have two children, Victor and Kenneth.

Lynda Mitchell (Social Services Director)
After graduating high school I attended college with the intention of pursuing a career in commercial arts. I moved to Boston and was there employed by a major electronic company and soon refocused my interest to industrial mechanics. I worked there as a technician for several years. While in Boston I also took an interest in the culinary arts and attended culinary school on the side. So today I am able to cook a great meal and repair my own kitchen appliances if need be. In 1990 I moved to Idaho and began working at this facility as a CNA. I have also worked in the medical records department. I have been with Advanced Health Care in Social Services since its inception. Today I still enjoy cooking along with amateur astronomy and scenic photography. I am the proud mother of one beautiful and intelligent daughter and the grandmother of two equally as beautiful and intelligent granddaughters.

June Aiken (Bookkeeping / Accounting)
I was born and raised on a farm in Keuterville, Idaho. I am married and have four children and two grandchildren. When not working in the nursing home I watch grandchildren or help my husband installing or refinishing floors. I have worked in nursing homes since 1986, the first five years as a CNA and since then in the Business Office.

Freida Holt (Business Office / Accounting)
I work at GH&R in Central Supply /Business Office. I have worked in healthcare for 23 years in various positions including CNA, Rehab Aide, Social Services Designee, Activities Aide, and Medical Records. I began working with Advanced Health Care at Idaho County Nursing Home and transferred to Grangeville Health & Rehab after the nursing home closed. I am married with five children and 16 grandchildren. I love the nursing field because I love people.
