Leslie Carlson, RN, Director of Nursing
I have an associate degree in nursing with eleven years of experience. I have worked at Aspen Transitional Rehab since February of 2006. I have three daughters and two grandsons.

Teresa Perez, Director of Dietary Services
I am a certified dietary manager with ten plus years of experience. I am from a very large family and am number 17 of 19 children, so our home was always busy. Growing up with many siblings we did a lot of sharing of responsibilities and helping each other. Our parents taught us that the greatest gift you can give someone is your time. We all ended up with different careers and I feel blessed to have been given this opportunity to work here at Aspen and have the privilege to serve our clients. It has truly been an honor to spend time with these clients and to provide them with something they enjoy, "great food". I have had several defining moments in my life and have accepted this area of service for the joy, simplicity and great achievement that I experience. I have really enjoyed the personal satisfaction of the warm acceptance I have felt here.

We have one child…a beautiful, curly, tow-headed little girl. I enjoy golfing, camping and spending time with family. I have worked for Advanced Health Care on a full-time basis for nearly four years and also provided consultation services for two years prior. As the Resource Utilization Coordinator, I spend a majority of my time completing government/Medicare required assessments, but you will also find me helping in a variety of other areas as needed.
